Wellbeing Service

Layton Medical Centre has always been excellent at dealing with physical health problems. We’re now able to give excellent support for people with mental health issues.

So if you are concerned about your wellbeing and would like to have a trained Practitioner listen to you, and to help identify what the problems might be and identify actions that will help you, please take advantage of our Wellbeing Service.

You can access the Wellbeing Service directly by asking for an appointment at reception – just ask for a first appointment with our Wellbeing Practitioner.

The Wellbeing Practitioner can help with a full range of wellbeing issues, including but not limited to:

  • Depression
  • Anxiety
  • Sleeplessness

If you wish you can complete one or both of our assessment questionnaires before you see the Practitioner, as this will enable them to understand your situation quicker. Just click on the links below to access a printable version of the forms or to submit your results to us securely on-line.
