Named Accountable GP

Layton Medical Centre is required to allocate a named, accountable GP to all patients, including children, Previously this has only been required for patients aged 75 or over.

Our approach is to assign patients to our GPs as follows:

  • Surnames from A-M are allocated to Dr Clark

  • Surnames from N-Z are allocated to Dr Shaw

  • There will be no change for patients aged 75 or over who have already been allocated a named, accountable GP, except where that GP has left the practice in which case the above approach will apply

If you prefer to be assigned a different GP to that which has been assigned, please make this known to the Practice Manager and reasonable efforts will be made to accommodate your request.

Your named GP is responsible for your overall care at the practice, and will take lead responsibility for the coordination of all services required under the contract and ensure they are delivered to each of their patients where required (based on the clinical judgment of the named accountable GP).

In addition, for patients aged 75 and over, the named accountable GP will also:

  • work with relevant associated health and social care professionals to deliver a multi-disciplinary care package that meets the needs of the patient

  • ensure patients have access to a health check as set out in section 7.9 of the GMS Contract Regulations.
